Welcome to the SusQ-Cyber Charter School 2017-2018 E-rate Household Survey. This survey is used to determine our E-rate discount that is applied towards Internet access and technology equipment. The information collected in this survey is used exclusively for E-rate purposes. SusQ-Cyber Charter School is required to collect names and other identifying information as part of the survey collection process. Names and identifying information will not be submitted as part of the information shared with E-rate, this is needed only to ensure the integrity of the survey. This survey and the information collected will be considered confidential and will be destroyed after the retention requirements have been met.

Please use the information in the above chart when considering your response to the questions in the survey.
To use this chart, locate your household size (including all adults and children living in the household). Once you have located your household size, locate the column that corresponds with your annual household income, or the number of times per month that you are paid. For example, if my household size is 3 total people and I am paid 2 times per month, I would use the amount of $1,235 when considering my responses to the survey questions.
If my household size is greater then 8 members, locate the column that corresponds to the number of times per month that you are paid in the "Each Additional Family Member Add" column and add that number to the corresponding column in the 8 household members row. For example, if my household is made up of 11 total people and I am paid once per week; I would multiply $148 by the number of people in my household over 8, in this case 3 additional people. $148 X 3 = $444. I would then move to the 8 household members row and find the column corresponding to being paid once per week, which is $1455. I would then add $1455 + $444 = $1899. I would use the amount of $1899 when considering my responses to the survey questions.
